Approach

Dr. Zelken's Philosophy

Facial enhancement should look like architecture, not addition — structure that belongs to your face, not something laid on top of it. At the Zelken Institute for Aesthetic Medicine in Newport Beach, board-certified plastic surgeon Dr. Jonathan Zelken approaches custom facial implants as a design problem solved one patient at a time.

Rather than reaching for a stock implant and hoping it fits, he studies your CT imaging and directs the design of an implant milled or 3D printed to your exact skeleton. The result is a hand-in-glove fit that reads as bone, not hardware.

Dr. Zelken's hands-on role in design and in-house planning gives him unusual control over contour, projection, and symmetry — the details that separate a natural result from an obvious one. His goal is the same one that guides all of his work: refinement that looks entirely your own, never overdone.

The Procedure

What Are Custom Facial Implants?

Custom facial implants are a precise, surgeon-directed approach to facial enhancement, offering symmetry and long-term stability that stock implants and fillers cannot match. Using a CT scan of your face, Dr. Zelken works with engineers to build a 3D model of your ideal structure, then designs an implant tailored to your unique anatomy — correcting asymmetry, restoring balance, and creating natural, harmonious contours.

His hands-on approach to design and in-house planning allows real control over fit, contour, and surgical outcome. Custom implants are commonly milled or 3D printed from PEEK (polyetheretherketone) or crafted from silicone. PEEK offers exceptional strength, biocompatibility, and radiolucency, and can be shaped for an exact "hand-in-glove" fit to the underlying bone.

For patients who prefer a faster or more affordable route, Dr. Zelken also offers a full range of off-the-shelf silicone implants for the chin, jaw angle, and cheeks. Whether custom or prefabricated, every option is chosen to suit your anatomy and your goals.

Featured Options

Facial Implants We Offer

Dr. Zelken offers both custom and off-the-shelf implants for the jaw, chin, and cheeks — tailored to your anatomy and goals.

  • Jaw Angle Implants Add lengthening, widening, and forward projection for a bolder, more defined jawline. Custom designs correct specific asymmetries or deliver more change than standard implants allow.
  • Chin Implants A strong chin adds balance and complements other features. Silicone implants are secured through a small incision; custom and off-the-shelf options suit your desired level of enhancement.
  • Cheek Implants Improve mid-face contours for a youthful, lifted appearance. We offer Malar Contoured and Orbital Rim designs for targeted enhancement around the eyes and upper face.

Is It Right For You

Who Is a Good Candidate?

Custom facial implants are well suited to patients with unique anatomical needs — congenital asymmetries, a desire for a stronger or more defined jaw angle, or facial imbalance following previous jaw or orthognathic surgery. Because each implant is designed around your own skeleton, it can address concerns that off-the-shelf implants simply aren't shaped to correct.

Good candidates are in good general health, understand the design timeline, and are looking for a definitive, long-lasting change rather than a temporary one. That said, not everyone needs a custom implant — for minor augmentation or a quicker turnaround, Dr. Zelken's prefabricated silicone options may be the better fit.

The right path — custom or off-the-shelf, PEEK or silicone — is determined together during your consultation, once Dr. Zelken has evaluated your anatomy and understood your goals.

What to Expect

The Custom Implant Process

Every custom implant begins with a personalized consultation. Dr. Zelken discusses your goals and evaluates your facial structure, then takes a CT scan to create a detailed 3D model of your facial skeleton.

In the design phase, he collaborates with expert engineers to shape an implant that enhances and balances your features — whether the plan calls for jaw lengthening, chin projection, or cheek augmentation. Each implant is planned around your specific anatomy so the finished result looks and feels natural.

Once the design is finalized, it is sent to a state-of-the-art facility for fabrication in PEEK or silicone, materials chosen for their compatibility with facial bone and their ability to be securely anchored. The design and fabrication process typically takes about two to three weeks. When your implant is ready, Dr. Zelken places it through carefully planned incisions in an accredited surgical facility — and because it is built to your skeleton, it seats precisely, forming the foundation of a refined, durable, and anatomically accurate result.

The Outcome

Results & Longevity

Custom facial implants deliver enduring results. Because the implant is milled or printed to fit your skeleton precisely and anchored securely, it provides stable, long-lasting structure rather than the temporary effect of fillers. PEEK and silicone are both durable, biocompatible materials with a long track record in facial and craniofacial surgery.

The hallmark of Dr. Zelken's work is restraint: the aim is not to make you look different, but to bring your features into balance so the change reads as natural harmony rather than obvious enhancement. Well-designed implants integrate with your existing anatomy, giving definition and symmetry that hold up over time.

Transparency

Risks & Considerations

As with any surgical procedure, facial implant placement carries risks such as swelling, bruising, infection, shifting, or asymmetry. It is also important to know that custom PEEK implants are not FDA-approved for aesthetic facial use, though PEEK has been used for years in craniofacial and reconstructive surgery. Dr. Zelken believes in complete transparency and will review every consideration, along with detailed pre- and post-operative instructions, during your consultation.

Common Questions

Custom Facial Implants FAQ

Common questions about custom facial implants, answered by Dr. Zelken.

The design process takes roughly two to three weeks from start to finish. Once finalized, your implant is fabricated and ready for surgical placement.

We use PEEK and silicone. Both offer exceptional durability, biocompatibility, and a natural feel, and both are well suited to the facial skeleton.

Custom implants suit those with unique anatomy, congenital differences, or a need for precision. Off-the-shelf options work well for minor, general enhancement — determined during your consultation.

Custom PEEK implants are not FDA-approved for aesthetic facial use, though PEEK has a long, well-documented history in craniofacial and reconstructive surgery. Dr. Zelken will review this fully with you.

Fillers add volume temporarily and soften over time. Custom implants provide permanent structural support built to your skeleton, delivering stable definition and symmetry that fillers cannot maintain long term.
